modifying decree re: tax dependent exemption

My divorce was granted in CT. I now live in MI with the child, noncustodial parent is still in CT. At time of divorce, he was given right to claim child as his tax dependent. Due to changes in the tax law, and to increases in the child's needs and expenses, I would like to modify this agreement so that I can claim the child as my dependent for income tax purposes.

1. How to I proceed if he agrees? What form do I file, how, and where?

2. If he does not agree, where do I address this issue, in CT or in MI? How do I do this from out of state?
